Membership Terms & Care Team Policy

The EndoAge Membership Program is designed to provide ongoing preventive, longevity, hormone, metabolic, and lifestyle support through a multidisciplinary care model. Membership aims to support long-term health optimization, education, accountability, and personalized health planning.

EndoAge memberships are not intended to replace:

• Emergency medical services
• Primary care services
• Specialist medical care
• Hospital-based treatment

Membership does not guarantee diagnosis, treatment, prescription access, health improvement, disease prevention, increased lifespan, or any specific clinical outcome.

Your care team may include:

• Physician (MD) — clinical oversight and consultations
• Health Coach — lifestyle, accountability, behavior change
• Administrative Coordinator — scheduling and administrative support

Care team composition may vary depending on your membership tier and clinical needs.

Consultations are conducted via secure video conferencing. Cancellations must be made at least 24 hours in advance. Late cancellations or missed appointments may be charged. Appointment availability is subject to physician schedules and may vary.

All clinical decisions are made exclusively by licensed physicians. Health coaches do not diagnose conditions, prescribe treatments, or provide individualized medical advice. Clinical decisions always involve qualified medical supervision.

EndoAge focuses on preventive, longevity, and optimization medicine. Services do not include management of acute or emergency medical conditions. If an acute condition arises, patients will be referred to appropriate local care.

Prescribing decisions remain solely at the discretion of the treating physician based on clinical evaluation, laboratory findings, risk assessment, and applicable regulations. Membership does not guarantee access to any specific medication, hormone therapy, weight-loss therapy, peptide therapy, or prescription renewal.

Laboratory tests may be ordered through independent third-party laboratories. Laboratory fees may be separate from membership fees. EndoAge is not responsible for laboratory delays, errors, or processing issues caused by third parties.

Wearable device data and AI-generated reports are supportive tools only. The EndoAge Score, AI summaries, and roadmaps are educational in nature and are not substitutes for physician evaluation or medical diagnosis. All AI-assisted outputs intended for clinical use are subject to physician review.

• Provide accurate and complete health information
• Disclose all medications, supplements, and relevant medical history
• Attend scheduled consultations or cancel with appropriate notice
• Seek local emergency care when advised
• Update health information when changes occur

Memberships renew automatically unless cancelled prior to the renewal date. Membership fees are generally non-refundable once services have commenced. Refund rights under applicable consumer protection laws remain unaffected.

EndoAge reserves the right to modify membership tiers, pricing, included services, and care team policies. Changes will be communicated via email or patient portal with reasonable notice. Continued membership after notification constitutes acceptance of the revised terms.
